Project L.O.V.E. Brochures
Project L. O. V. E. has launched an exciting new program called the Immunization Peer Advocate Program (IPA).
This new program is a combination of education and case management to try and eliminate any barriers that may prevent you from getting your baby vaccinated on time, whether that be just education and answering questions, or assistance with transportation to and from clinic appointments. We want to be there for you when and if you need us. There are incentives for participation.
To participate, you must:
1. reside in 43211 or 43203
2. Be currently pregnant or
3. Be the primary caregiver of a child under 2 years of age
If you are an interested parent, call us directly at 614-645-1478 to sign up!!
If you are a community agency, you can refer eligible parents directly to us by:
1. Hanging this flyer on your organization's information board where parents can see,
and/or
2. Calling us at 614-645-1478 and providing us with their contact information,
3. Filling out a referral sheet and faxing it over to us at 614-645-0006,
